Surgical Services Terms of Service
Effective January 1st, 2026
These Terms of Service govern the scheduling, payment, cancellation, performance, and related conditions of hair restoration procedures provided by Sara Wasserbauer MD, PC, doing business as California Hair Surgeon (“Practice”). These terms supplement, and do not replace, the Practice’s Procedure Agreement, informed-consent documents, privacy notices, and clinical instructions.
- Procedure Estimates and Scheduling
Procedure recommendations, estimated fees, and planned follicular graft goals are based on the information available at the time of consultation. Unless otherwise stated in writing, procedure estimates are valid for six months. A procedure date is not reserved until the required deposit has been received.
- Deposit and Payment
A $1,000 deposit is required to reserve a procedure date. The remaining balance must be paid no later than four weeks, or twenty business days, before the scheduled procedure.
Deposits are non-refundable within four weeks or twenty business days of the procedure date, except where otherwise required by law or expressly stated in writing by the Practice. Credit-card processing fees, financing charges, lender fees, and other third-party fees are non-refundable.
- Rescheduling
All rescheduling requests must be submitted in writing. A request made within four weeks, or twenty business days of the procedure date will result in a $5,500 rescheduling fee.
Patients may reschedule a procedure no more than two times. Additional rescheduling requests may require payment of the full remaining procedure balance before another date is reserved.
- Cancellations and Refunds
Cancellation requests must be submitted in writing.
A cancellation received at least four weeks or twenty business days before the scheduled procedure will receive a refund of amounts paid, excluding non-refundable processing fees, financing charges, and other third-party fees.
A cancellation received at least two weeks or ten business days before the procedure will receive a refund of 75% of eligible amounts paid, excluding the non-refundable deposit, processing fees, financing charges, and other third-party fees.
Cancellations made within two weeks, or ten business days of the procedure are non-refundable.
- Practice Scheduling Changes
The Practice may adjust a procedure date or time because of provider availability, clinical needs, safety concerns, staffing, equipment, or other operational requirements. When practical, the Practice will work with the patient to identify an alternative date.
- Follicular Graft Goal
The Practice will make reasonable clinical efforts to achieve the documented follicular graft goal. However, surgical findings, donor-hair availability, patient anatomy, safety considerations, graft quality, and other patient-specific or unexpected circumstances may prevent the Practice from reaching the planned number.
If the final graft count is below the documented graft goal, the patient will receive a $1,000 refund from the surgery fee. This refund is the Practice’s stated financial adjustment for not reaching the documented graft goal and does not constitute a guarantee of graft survival, growth, density, coverage, appearance, or any other result.
- No Guarantee of Results
Hair restoration is both a medical and cosmetic procedure, and individual results vary. The Practice does not guarantee any particular density, growth rate, coverage, appearance, hairline, graft survival rate, or cosmetic outcome.
Results may be affected by the patient’s age, health, medical history, scalp condition, donor-hair availability, hair characteristics, healing response, compliance with instructions, and progression of future hair loss. Photographs, simulations, diagrams, prior-patient results, and graft estimates are provided for educational and planning purposes and do not create a warranty or guarantee.
- Medical Risks and Informed Consent
All medical procedures involve risks. Before surgery, the patient will be given an opportunity to discuss the procedure’s purpose, expected benefits, material risks, alternatives, recovery expectations, and possible complications with the provider or clinical team.
These Terms of Service do not replace the patient-specific informed-consent process or any separate consent forms required by the Practice.
- Patient Responsibilities
The patient must provide accurate and complete information concerning medical history, medications, allergies, supplements, prior procedures, tobacco or substance use, and any changes in health before surgery.
The patient must follow all preoperative and postoperative instructions. Failure to provide accurate information or follow instructions may increase medical risk, affect graft survival or results, and result in the procedure being modified, postponed, or canceled.
- Clinical Judgment
The provider may modify, postpone, stop, or decline a procedure when, in the provider’s clinical judgment, proceeding would be unsafe, medically inappropriate, or contrary to the patient’s best interests.
If a procedure is postponed or canceled for clinical or safety reasons, the Practice will determine whether eligible amounts paid will be refunded or applied toward a future procedure based on the circumstances and applicable law.
- Medications and Transportation
When medications, anesthesia, sedation, or other substances may impair the patient’s ability to drive, the patient must arrange for a responsible adult to provide transportation home. The Practice may postpone or cancel the procedure if appropriate transportation has not been arranged.
- Photographs, Medical Records, and Privacy
The Practice may take and maintain photographs, measurements, imaging, and treatment records for medical documentation, treatment planning, and follow-up care. Patient information will be handled in accordance with applicable privacy laws and the Practice’s privacy policies.
Photographs will not be used for marketing, advertising, education, or promotional purposes without a separate written authorization when required.
- Third-Party Financing
Patients using third-party financing remain responsible for complying with the financing provider’s terms. The Practice is not responsible for lender fees, interest charges, declined financing, payment delays, or disputes between the patient and the financing provider.
Financing approval does not alter the Practice’s payment, scheduling, cancellation, or refund policies unless an authorized Practice representative agrees otherwise in writing.
- Written Communications and Modifications
These Terms of Service, together with the Procedure Agreement, informed-consent documents, and applicable Practice policies, constitute the agreement governing the procedure.
Statements made orally by staff do not modify the Practice’s payment, cancellation, rescheduling, graft-goal refund, or other financial policies unless confirmed in writing by an authorized Practice representative.
- Acceptance
By signing the Procedure Agreement, paying a deposit, or proceeding with treatment, the patient acknowledges that they have reviewed and accepted these Terms of Service.
